Incident Reporting

Please use this form to report any incidents or near misses. We collect this data to help us to support you as leaders, and to help us to learn and improve safety across the District.

Note that this form should be used in addition to your local Group / Unit reporting process, and any HQ requirements.


    Remember that you must report any incident requiring intervention by outside agencies, eg an ambulance or a GP. See the HQ Website for more information.
